Thank you for your interest in enrolling your child into Yew Chung International School of Qingdao (YCIS Qingdao). The following items must be completed and received by the school before an application can be processed. Documents can be submitted in person, by fax, email, or mail. Any questions? 1. YCIS Qingdao Student Application Form (completed in full, signed and dated) After completing the Electronic Application Form, please email it as an XML attachment to enquiry@qd.ycef.com. If you don’t have Adobe Reader, you may download now it for free at www.adobe.com/downloads/ .
2. For Year 2 to Year 13 applicants, please provide photocopies of school academic reports/records and any other relevant documentation for the most recent two years. (For Year 1 applicants and below, please include any school reports/records, if available). These reports/records must be in either English or Chinese, translated if needed by an authorised agency.
3. Payment of the Application Fee (non-refundable) of RMB 3,000 yuan by:
   
   • Cash (in RMB only); or
   • Electronic/bank transfer (in RMB or USD) according to the information in the YCIS Qingdao Fee Schedule document.
     Note: Credit card and personal cheques cannot be accepted.
4. One passport-sized photo for each child with the passport name of the child neatly printed on the back of each photo.
5. Photocopies of the passport and visa page of both the child and the parents.

     • Hong Kong Permanent Identity Card,
     • Passport
     • Hong Kong / Macau Staff Employment Permit
   • Taiwan residents: photocopies of the
     • Mainland Travel Permit for Taiwan Residents
     • Taiwan Staff Employment Permit

Once all of the above items have been received by the school, we will contact you regarding any further information or assessment needed for your child. After we have completed our review, we will issue you a decision promptly.
